OUR OFFICE POLICIES

In order to acquaint you with our office and policies, we provide our patients with this introduction. We welcome you and thank you for selecting our office to manage your dental health. Our staff is dedicated to make your visits as pleasant as possible. Your complete satisfaction is our goal.

NEW PATIENTS
Every new patient is asked to fill out an updated medical and dental history. This is required in order to provide the most appropriate care for patients with certain medical conditions. Please be as thorough as possible when filling out these forms.

INSURANCE
Dental insurance was never intended to cover the entire cost of treatment. Most plans include co-payments, deductibles, limitations and exclusions. While we participate with many insurance plans, please be aware that your employer selected your insurance benefits. We will be more than happy to access and verify your benefits. We will work with you to assure that you receive the maximum benefits available under your specific plan.

PAYMENT AND FEES
In order to eliminate the expense of billing and collection, all out-of-pocket expenses must be paid at the time of service. Dentures, partials, cosmetic treatment, crowns and bridges must be paid in full at time of delivery. Surgical services (extractions, etc) must be paid in full at the surgery appointment. A $25 charge will be assessed if an appointment is broken (No show or missed appointment) and we do not receive 24 notice.
